Facial paralysis presents profound functional and psychosocial challenges, prompting ongoing innovation in reconstructive surgery. Over the past several decades, facial reanimation has evolved from static interventions, such as brow lifts and gold weight implantations, to dynamic techniques aimed at restoring natural movement and expression. Early nerve transfer procedures, including cross-facial nerve grafts and hypoglossal-facial nerve anastomoses, laid the foundation for contemporary microsurgical approaches. Free functional muscle transfers, particularly gracilis transplants, have emerged as gold-standard strategies, offering voluntary and spontaneous movement when combined with cross-facial nerve grafting. Advances in surgical planning, intraoperative nerve monitoring, and postoperative rehabilitation have further enhanced outcomes, improving symmetry, tone, and patient satisfaction. More recently, minimally invasive and hybrid techniques integrating bioengineering and regenerative medicine hold promise for reducing morbidity while optimizing functional recovery. This review traces the trajectory of facial reanimation, highlighting key innovations, comparative outcomes, and future directions.
