Purpose To demonstrate the benefits of a proactive GSGS model for older surgical patients (>65yrs) Methodology 3 month pilot 3 ward rounds weekly by a Consultant Geriatrician and full time geriatrics registrar Proactive, comprehensive geriatric care provided for all new >65 yrs emergency laparotomy (EL) patients admitted to surgical wards. Additional older patients referred by surgical teams also reviewed. Data collected on medical interventions, length of stay and discharge destination. Staff satisfaction surveys completed. Results 94 patients in total were reviewed over the 3 month period 39/94 patients reviewed met criteria for the Australia and New Zealand Emergency Laparotomy Audit (ANZELA-QI). Mean age 80.4 yrs. Mean Clinical Frailty Scale = 5. Mean Edmundson Comorbidity index = 6. 50% of patients had an abnormality in geriatric blood screen identified and treated. 66% had chronic disease optimisation and 42 % had new chronic disease diagnosed. 75 % had a new medical complication diagnosed and managed. 18.1% received delirium management. Goals of care completed by GSGS in 87% of patients. Average LOS of new ANZELA-QI patients 11.56 days. 30/94 patients wait listed for inpatient rehabilitation of which 27% were transferred the same day. Average time for waitlisting from surgical clearance 0.5 days. Unanimous support for permanent service on staff survey. Conclusion This pilot demonstrates the far reaching benefits of a collaborative GSGS care model, from providing medical optimisation and patient advocacy to supporting junior doctors and improving bed flow; LOS 11.56 days for ANZELA-QI patients versus 13.7 days (2023/24 ANZELA-QI pre-pilot data). An additional noteworthy benefit includes accurately capturing patient complexity to facilitate correct clinical coding. Proactive geriatric care will be one of the new ACSQHC 2026 Emergency Laparotomy Clinical Care Standards for older patients and as such hospitals are urged to prioritise this as cost effective service development.
