Background
Cataract extraction was the most commonly performed elective surgical procedure in Australia in 2023-24 (1) Since Sir Harold Ridley’s first intraocular lens (IOL) implantation in 1949 (2), a number of major innovations have transformed cataract surgery into a precise and highly refined refractive procedure. In addition to restoring vision, advancements in IOL materials, haptic configuration and optical engineering have enabled cataract surgery to optimise individual refractive outcomes and tailor visual performance to patient needs.
Methods
A narrative review of the evolution of IOL implants was conducted, with a focus on lens materials, biocompatibility, fixation design and optical performance.
Results
Early IOLs were rigid, sharp-edged and constructed from polymethyl methacrylate (PMMA), requiring large corneal incisions and often resulting in inflammatory complications and dislocation (3). The subsequent adoption of loop-haptic designs improved centration and reduced mechanical trauma, while the introduction of foldable silicone and acrylic materials allowed for IOLs to be delivered through smaller, self-sealing incisions (4, 5). More recent advancements include toric IOLS capable of correcting astigmatism, multifocal and extended-depth-of-focus (EDOF) IOLs providing both distance and near vision without the need for spectacles (6, 7).
Conclusion
Progressive improvements in IOL materials and design have significantly enhanced safety, stability and optical performance. Modern implants now allow surgeons to achieve predictable refractive targets and optimise visual function for individual patients.
