Purpose
Acute appendicitis is a common emergency surgical presentation in NSW. Existing appendicitis scoring systems, such as the Alvarado score, focus on diagnostic likelihood, but do not define disease severity or predict postoperative length of stay (LOS).1 We aimed to develop a novel severity-based scoring system, the CAMP Score (Campbelltown Appendicitis Morbidity Prognostic Score), to predict LOS following emergency appendicectomy.
Methodology
A retrospective analysis was performed using an ethics-approved database at Campbelltown Hospital, including 300 patients who underwent emergency appendicectomy. Preoperative variables including age, ASA score, white cell count, and C-reactive protein, as well as intraoperative variables including operative duration, conversion to open surgery, drain usage, base of appendix involvement, and macroscopic appendix appearance, were analysed for their association with length of stay. Statistically significant variables were incorporated into the CAMP Score. Preoperative variables and operative duration greater than 60 minutes were allocated 1 point, while all other intraoperative variables were allocated 2 points.
Results
Multiple preoperative and intraoperative variables were significantly associated with increased length of stay. A CAMP Score less than 2 predicted a LOS of 1 day or less. Scores between 2-9 were associated with a LOS of 2-5 days, while scores of ≥ 10 predicted a LOS of ≥10 days.
Conclusion
The CAMP Score is a simple, clinically applicable tool that stratifies appendicitis severity and predicts postoperative length of stay. It may support perioperative decision-making, discharge planning, advice to parents and patients, as well as guide antibiotic duration and inform the development of health economic models in general surgery departments.
