Changing trends in age of presentation of Acute Cholecystitis

Institution: Hutt Hospital - Wellington , Aotearoa New Zealand

Purpose Acute Cholecystitis is viewed traditionally as a condition affecting patients over 40 years of age2. However, there is a downwards trend in presentation age, with an increase in patients under 30 years of age requiring surgical management. We aim to investigate the shift in presentation age of Acute Cholecystitis requiring surgical intervention, the causes effecting this change and the impact on healthcare. Methodology Retrospective chart review of patients presented with Acute Cholecystitis and acutely treated with Laparoscopic Cholecystectomy in a Single Centre from 2023 to 2025 in New Zealand. Demographics and length of stay (LoS) was investigated. Results In total 165 patients met the inclusion criteria, of these 136 were 30 or older, 29 were younger, accounting for 17.5%. The youngest patient was 16 and the oldest 89-year-old. The average length of stay was 5 days for the over 30 group and 3 days for the under. An unpaired T test was performed which demonstrated statistically significant difference for LoS with a P value of 0.005. In the over 30s there were 58 Males and 78 Females while in the under 30s there were 4 Males and 25 Females. A Chi squared test with Yates correction demonstrated statistical significance in the difference in patient genders between the two groups with a P value of 0.0069. Conclusion 17.5% of the cohort treated for Acute Cholecystitis were under 30 years of age, this is significantly higher than the 1.8% presented in published literature5. The cause of this is likely multifactorial and requires further research. A multi-centre study would be beneficial to investigate this change in age of presentation and its causes over time especially as there is projected increased global burden of Cholecystitis by 20404.
