Abdominoperineal resections (APRs) are commonly performed for low rectal cancers, as well as benign conditions, such as inflammatory bowel disease. The perineal component of this operation is typically performed via an open approach and, particularly the anterior portion of this dissection, can be technically challenging due to the high risk of genitourinary injury. A simple strategy to reduce this risk is to perform the anterior dissection last and deliver the specimen through the perineal wound in order to provide greater traction on its anterior attachments. However, this step itself can be difficult in the case of a bulky specimen and attempting to deliver it may risk fracturing the specimen. We describe and illustrate the use of a Foley catheter as a sling around the specimen to provide gentle traction, facilitating its safe delivery through the perineal wound. This technique has never been reported in the literature. Although we describe its use in an open case, this technique can also be used in laparoscopic or robotic approaches.
