Minimally Invasive Liver Resection: Peri-operative Outcomes from Two Specialist Units

Institution: Austin Hospital - Victoria , Australia

Background: Minimally invasive liver surgery (MILS), including laparoscopic and robotic approaches, is increasingly utilised in hepatobiliary surgery, with advantages including shorter length of stay, reduced morbidity and improved surgical outcomes compared with open surgery. Despite these advantages, MILS has not been widely adopted as standard practice in Australia with limited local literature. This study aimed to describe the contemporary practice of MILS and its outcomes in Melbourne. Methods: A retrospective analysis was conducted using prospectively maintained databases across both Austin and Alfred HPB units for all MILS performed between January 2024 and December 2025. Demographic data, operative data, and postoperative outcomes were analysed. Results: Fifty-seven patients underwent MILS, including 25 robotic and 32 laparoscopic resections. 15 patients (26%) had underlying cirrhosis. The median age was 69 (IQR 61-75.5) vs 64 years (IQR 54.25-69.25) for robotic and laparoscopic resections, respectively (p=0.09). The median Charlson Comorbidity Index was 7 (IQR 3-9) for the robotic cohort compared with a median of 5 (IQR 3-6) for the laparoscopic cohort (p=0.07). Median IWATE score was higher in the robotic cohort at 8 (IQR 5–10) compared with 4 (IQR 3–5) in the laparoscopic cohort (p<0.001). Median operating time in the robotic cohort at 362 minutes (IQR 315–451) compared with 197 minutes (IQR 134–267) in the laparoscopic cohort (p<0.001). Major morbidity (Clavien-Dindo ≥III) occurred in three patients in each group, with one 30-day mortality in the laparoscopic cohort. Conclusion: Minimally invasive liver resections can be safely performed with acceptable peri-operative outcomes, including in complex and high-risk patients. Robotic liver resection appears to be utilised even for higher-complexity cases and demonstrated comparable major morbidity to laparoscopic surgery.
