Background
Defining melanoma margins is a recognised challenge particularly in lentigo maligna (LM) and lentigo maligna melanoma (LMM), due to subclinical extension and poorly defined clinical borders. In aesthetically sensitive subunits of the face, accurate delineation of surgical margins is especially important in terms of function and aesthetic.
The use of ultraviolet illumination from the Wood’s lamp has been described to enhance the contrast of melanoma pigmentation. This could serve as an adjunct to demarcate surgical margins for LM and LMM. However, there is no current literature evaluating its effectiveness in this context.
This study aims to evaluate histological margin clearance before and after the implementation of the Wood’s lamp in pre-operative marking of LM and LMM in the head and neck region.
Methods
This retrospective quasi-experimental study was based data from a single plastic surgeon based in Far North Queensland. We reviewed and analysed data from all patients who underwent melanoma excision in the head and neck region from April 2021 to March 2025. The use of the Wood’s lamp in the pre-operative marking for melanoma was introduced in April 2023. Chi-Squared Test were used for data analysis, with a significance level of p<0.05.
Results
31 patients were identified based on inclusion and exclusion criteria. Histological margins were compared between the control (n=14) and intervention (n=17) groups. There was no statistical difference observed in excision margin outcomes following the implementation of the Wood’s lamp.
Conclusion
There was no statistically significant difference in excisional margins for LM and LMM in the head and neck region following implementation of the Wood’s lamp in pre-operative marking of melanoma in this single surgeon study.
